Being able to enter mobile or email address for repeats

Is it possible to get MedView Flow to include a field where the patient either enters their mobile or email address for repeats? Ideally if there are repeats patients shouldn't be able to submit their scripts until selecting their preference

    We have setup the MedView Flow app so that patients scan their own QR codes however there is no option for customer preference for where they want repeats sent to. A mobile/email/print option should be included so that customers can confirm their preference on repeats. Ideally if there are repeats patients shouldn't be able to submit their scripts until they have selected an option. The UI of the notes section is not really conducive with this as you can't see what you're typing once the keyboard pops up on an iPad.
